Hospice Services

Hospice care is provided through an interdisciplinary, medically directed team. This team’s approach to care for patients with life-limiting illnesses includes a physician, a nurse, a hospice aide, a social worker, a chaplain, and a volunteer. The hospice nurse makes regular scheduled visits to patients in order to provide expert pain and symptom management and control. Throughout the time that the patient is under our care, the nurse keeps the primary care physician informed of the patient’s condition. Nurses provide the complete spectrum of skilled nursing care. Nurses are available on-call, 24 hours a day, seven days a week.
